In an aerospace manufacturing environment, the people authorized to initiate a nonconformance report are defined by the organization’s approved quality management system, customer requirements, and system permissions. There is no universal aerospace rule that only one role may initiate an NCR. Commonly, trained inspectors, quality engineers, production supervisors, manufacturing engineers, operators, receiving inspection, supplier quality, and sometimes customer or source inspectors may raise or initiate an NCR, depending on the site procedure.
The practical distinction is important: many organizations allow any trained employee to identify and report a suspected nonconformance, but they restrict formal NCR creation, classification, containment approval, and disposition to specific authorized roles.
Initiating is not the same as dispositioning
Opening an NCR does not normally mean the initiator is authorized to decide what happens to the part, assembly, material, tool, process output, or record. Disposition authority is usually more tightly controlled.
For example, an operator may be allowed to flag a suspect condition and start the NCR workflow. A quality inspector may verify the condition and document objective evidence. A material review board may be required to approve use-as-is, repair, or scrap decisions. Engineering, the customer, or the design authority may be required for departures from design requirements, depending on contract and regulatory context.
This separation protects traceability and prevents informal acceptance of nonconforming product.

Most aerospace manufacturers define NCR initiation authority in procedures, role matrices, training records, or electronic workflow permissions. Typical controls include:
- Training requirements before a user can create or submit an NCR.

- Required fields for part number, serial or lot number, operation, drawing requirement, defect description, and objective evidence.

AS9100-type quality systems expect control of nonconforming outputs, but the standard does not by itself assign one job title as the NCR initiator. The enforceable answer is normally found in the site’s documented process and any applicable customer flowdowns.
Where brownfield systems create risk
In older plants, NCR initiation may happen across paper travelers, MES terminals, ERP quality modules, supplier portals, spreadsheets, and standalone QMS tools. That can create gaps if the person who detects the issue cannot access the system of record, or if the NCR does not link cleanly to the work order, inspection result, serial number, purchase order, or drawing revision.
Replacing all legacy systems just to standardize NCR initiation is usually unrealistic in aerospace-grade environments. Qualification burden, validation cost, downtime risk, integration complexity, long asset lifecycles, and traceability obligations often make controlled coexistence the more practical path. The important control is that the approved process defines where the official NCR record lives, who may create it, and how related systems stay synchronized.
Common failure modes
The main risk is not that too many people notice nonconformances. The larger risks are poor authorization boundaries, weak evidence capture, and unclear disposition authority.
- Operators can see defects but are not allowed or trained to record them, so issues move informally.
- Supervisors bypass formal NCR initiation to protect schedule metrics.
- Duplicate NCRs are created in separate MES, ERP, or QMS records without reconciliation.
- Initiators select the wrong part, operation, revision, or serial number.
- Containment is assumed but not documented.
- Disposition is made by someone without delegated authority.
- Customer or regulatory notification requirements are missed because escalation rules are unclear.
Practical answer
The authorized NCR initiator is whoever the site’s controlled procedure and workflow permissions say it is. A mature process usually makes it easy for trained personnel to report suspected nonconformances, while reserving verification, classification, containment approval, MRB disposition, corrective action, and closure for defined roles with appropriate authority.
If the procedure is ambiguous, the safe operational assumption is not to rely on job title alone. Check the controlled QMS procedure, training matrix, customer requirements, and electronic access rules before treating an NCR as formally initiated.
