AS9102 Form 3 is the First Article Inspection form used to document characteristic accountability, verification results, and related evaluation for an aerospace part or assembly. It lists the design characteristics that must be verified and records the measured or observed results for each characteristic.

In an AS9102 First Article Inspection Report, Form 3 is commonly tied to a ballooned drawing or other indexed design record. Each listed characteristic is typically associated with a unique characteristic number, the requirement, the inspection result, and the acceptance status or applicable remarks. Characteristics may include dimensions, tolerances, notes, material callouts, process requirements, and other design requirements that need objective evidence.

Form 3 should not be confused with AS9102 Form 1, which addresses part number accountability, or AS9102 Form 2, which addresses product accountability such as materials, special processes, and functional testing. Form 3 is focused on verifying and recording individual design characteristics as part of the FAI record.
